The Social Services department at The Curtis Home plays a key role in helping you or your loved one adjust to our facility. Whether rehabilitation and returning to the community is your goal, or staying in our facility long-term, the social service staff is there to support you every step of the way. Our Social Worker will assist with achieving emotional security and comfort, individuality, dignity, privacy and an overall sense of well-being. They achieve this goal by completing a comprehensive psychosocial assessment of each resident’s individual needs while working in collaboration with the rest of the interdisciplinary team to develop and implement an appropriate plan of care.
For short-term residents returning to the community, social services will arrange homecare, order any needed equipment and services, and ensure a smooth transition to the home environment.
